Licensed Practical Nurse (LPN) - Titusville Nursing and Rehab

Responsibilities:
Provide routine nursing care to patients, including administering medications, performing vital signs, and providing wound care
Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
Monitor patient conditions and report changes to healthcare providers
Assist with patient transfers and ambulation
Participate in interdisciplinary team meetings to discuss patient care and progress
Maintain accurate and up-to-date patient records and medication charts
Collaborate with other healthcare professionals to ensure seamless patient care and communication
